Your Responsibilities:

  • Providing face to face (and occasional) tele-support to clients in both a community and clinical setting 
  • Providing exercise & behaviour change coaching to clients living with disability or chronic/complex conditions
  • Planning, implementing, and evaluating services for Rebound’s clients
  • Providing appropriately targeted education for clients, carers and staff as required
  • Communicating and working efficiently and collaboratively with the team to ensure best practice & integrated client care
