The Rebound Health Veterans’ Health program is a collaborative approach of Dietitian, Exercise Physiology, Physiotherapy and Occupational Therapy services for DVA clients. 

What is the Veterans’ Health Program?

Entitled Department of Veterans’ Affairs (DVA) clients may be referred for clinically necessary treatment by their general practitioner on a valid D904 referral form.

Gold card holders are entitled to clinically necessary treatment covered by DVA’s health care arrangements for all health conditions. White card holders are entitled to clinically necessary treatment for an accepted condition.

These specific medical conditions may include: 

Heart disease / hypertension 

PTSD / mental health

Osteoarthritis

Obesity  


What to Expect

We provide practical food-based recommendations, strategies and ongoing support to assist you toward your health goals.  

Initial assessment: 30 minute 1:1 consultation with an Accredited Practising Dietitian (APD) discussing your personal health, dietary and lifestyle history and collaboratively working on a plan to assist in achieving your health and nutrition goals

Follow-up consultations: you are eligible for 12x 30-minute sessions per treatment cycle discussing and modifying goals, providing nutrition education, assessing nutrition intake, and strategies to aim to implement. 

Meal planning: under DVA you can also claim services for the development of personalised menu plans to assist in the practical implementation of dietary recommendations at home.

  • Yes, you will need a D904 referral from your GP to engage in Dietitian services.

  • This is not essential; however, If you do have any recent blood tests, diagnostic testing or medical reports it will be beneficial for you to bring these along to your first appointment so we can understand your health journey to date.

  • This isn’t necessary; we spend the time in the initial appointment to discuss your eating habits and general day-to-day variabilities. Although if you would find this useful to remember your intake then please bring it along!
