How to Get Started
If you hold a DVA Gold Card or White Card, you may be eligible for funded services.
Getting started is simple:
Speak with your GP to arrange a DVA referral (D904 form).
Contact our team to book your first appointment.

Yes, you will need a D904 referral from your GP to engage in exercise physiology.
What should I bring to my initial assessment?
Please wear something you are comfortable moving in and your referral form
How often will I do exercise physiology sessions?
You can discuss with your exercise physiologist about what is most appropriate for you. It is common for clients to attend either once or multiple times per week or fortnightly.
