How can we help?

When a senior receives approval for a home care package (HCP), their care manager can organise for one of our exercise physiologists to come and perform an initial assessment with the client.

We can perform strength, mobility and balance training, along with working on skills such as transfers, walking, stair climbing and other activities of daily living.

We provide an individualised experience where we can work to a client’s schedule and build a management plan that suits their specific needs. 


Frequently Asked Questions

  • The term “home care package” refers to an amount of money allocated by the federal government to an older person to provide services which will enable them to continue to live independently.
    The amount of money provided depends on the person’s needs, as assessed by an independent assessment agency.
    There are certain packages older adults are eligible for, from basic care needs (Level 1) through to high care needs (Level 4).
    They can use these funds towards services such as social support, well-being services, home modifications as well as allied health services such as Exercise Physiology.

  • Usually once a senior has been approved for a HCP, they will sign up with a provider to help manage their funds on their behalf.
    The provider will assess the clients care needs and then source third parties to help provide the care like Rebound Health.

    When a client has a HCP, we would obtain the HCP case manager details and call them on behalf of the client to introduce our services.
    The HCP manager then sends us an agreement for our services or approves the Exercise Physiologist sessions for this client.
